Overview

Patient charts in AllCare provide providers with a comprehensive view of patient information. This includes demographics, medical history, current medications, visit history, and clinical documentation.
View Access: Providers can view patient charts for patients assigned to them or at their assigned facilities. Clinical documentation and prescribing are handled through the EMR system (eClinicalWorks).

Accessing Patient Charts

From Your Patient List

Navigate to Patients

Click Patients in the main navigation

View Your Patients

The list shows patients at your assigned facilities

Search or Filter

Use the search bar or filters to find specific patients:
  • Search by name
  • Filter by facility
  • Filter by status (Active, Inactive)

Open Patient Chart

Click on a patient’s name to open their chart

From Your Schedule

Go to Schedule

Navigate to your visit calendar

Select a Visit

Click on a scheduled visit

Open Patient Chart

Click the patient name or View Chart button

Patient Chart Sections

Demographics

The demographics section displays basic patient information:

Emergency ContactEmergency contact person and phone
InsurancePrimary and secondary insurance information
FacilityCurrent facility where patient resides
Patient demographics are managed by facility staff. Contact the facility if information needs to be updated.

Medical Information

Allergies

Drug allergies, food allergies, and environmental allergies with reaction details

Diagnoses

Active diagnoses with ICD-10 codes

Medical History

Past medical history, surgical history, and hospitalizations

Vitals

Recent vital signs (BP, pulse, temperature, weight)

Medications

The medications section shows all current and recent medications:

Medication Categories

Medications taken on a regular schedule (daily, twice daily, etc.)
As-needed medications with specific indications
Schedule II-V medications with DEA tracking
Medications stopped within the last 30 days
Prescribing: To prescribe or modify medications, use the EMR system (eClinicalWorks). AllCare displays medication information for reference.

Visit History

View the patient’s visit history:

The documents section contains patient-related files:

Click on any document to view or download. Documents can be filtered by type and date range.

View the patient’s care team:

Patient Status Indicators

Patient charts display status indicators:
IndicatorMeaning
🟢 ActivePatient is currently receiving care
🟡 PendingPatient enrollment in progress
🔴 InactivePatient no longer receiving care
⚠️ AlertPatient has clinical alerts (allergies, fall risk, etc.)

Troubleshooting

Possible causes:
  • Patient is at a facility you’re not assigned to
  • Patient status is inactive
  • Search filters are too restrictive
Solution: Check your facility assignments and clear any filters. Contact AllCare Operations if you should have access.
Possible causes:
  • Information not yet entered by facility staff
  • Data sync delay from EMR
  • Information in a different section
Solution: Check other sections of the chart. If information should be present, contact the facility.
Possible causes:
  • Recent changes not yet synced
  • Viewing wrong date range
Solution: Refresh the page and check the medication sync timestamp. Recent changes appear after the next sync cycle.
Possible causes:
  • Providers have view-only access to patient charts
  • Edits are made in the EMR or by facility staff
Solution: For clinical changes, use eClinicalWorks. For demographic changes, contact the facility.
