Overview
Patient charts in AllCare provide providers with a comprehensive view of patient information. This includes demographics, medical history, current medications, visit history, and clinical documentation.View Access: Providers can view patient charts for patients assigned to them or at their assigned facilities. Clinical documentation and prescribing are handled through the EMR system (eClinicalWorks).
Accessing Patient Charts
From Your Patient List

Navigate to Patients
Click Patients in the main navigation

View Your Patients
The list shows patients at your assigned facilities

Search or Filter
Use the search bar or filters to find specific patients:
- Search by name
- Filter by facility
- Filter by status (Active, Inactive)

Open Patient Chart
Click on a patient’s name to open their chart
From Your Schedule

Navigate to your visit calendar

Click on a scheduled visit

Click the patient name or View Chart button
Patient Chart Sections
Demographics
The demographics section displays basic patient information:| Field | Description |

Patient demographics are managed by facility staff. Contact the facility if information needs to be updated.

Allergies
Drug allergies, food allergies, and environmental allergies with reaction details
Diagnoses
Active diagnoses with ICD-10 codes
Medical History
Past medical history, surgical history, and hospitalizations
Vitals
Recent vital signs (BP, pulse, temperature, weight)
Medications
The medications section shows all current and recent medications:Active Medications

Patient Status Indicators
Patient charts display status indicators:| Indicator | Meaning |

| 🟢 Active | Patient is currently receiving care |
| 🟡 Pending | Patient enrollment in progress |
| 🔴 Inactive | Patient no longer receiving care |
| ⚠️ Alert | Patient has clinical alerts (allergies, fall risk, etc.) |
